DocuSign Forms
Before you initiate a DocuSign request below, create your DocuSign account via docusign.fiu.edu and authenticate using your FIU student single-sign-on credentials. You will only need to complete this process once .

Please Note: Documents initiated before creating your account will not be accessible within your account. However, all new requests generated after your account creation will be readily available on the DocuSign Manage page. Here, you can conveniently review both completed documents and those pending approval signatures. Additionally, you have the option to download documents. Do not forget to submit your completed documents for processing by following the submission instructions as included on the form.

      Submit Forms Online

      How to upload forms to MyFIU. Most OneStop forms can be submitted this way.

      Step 1: Complete Form

      Download your required form(s) above. Complete the form and make sure to collect any required signatures (if applicable).

      Step 2: Scan All Documents

      Scan your completed form(s) and/or supporting documents. Scanners are available on campus. Acceptable file types include: PDF, JPG/JPEG, GIF, PNG, BMP, DOC and TIF/TIFF.

      Step 3: Upload to MyFIU

      Use the upload tool on MyFIU:

      1. Log onto MyFIU.
      2. Select the 'Student Tools' tile.
      3. Click the 'Document Uploads' tile
      4. Review the instructions then click 'Accept and Continue.'
      5. Select the Department, Aid Year (if applicable) and Form Name from the drop down menus. Check the bottom of the form you are uploading if you are unsure of the department.
      6. Select your file and 'Upload.' Remember to upload any supporting documents too. Use the + and – to add or remove files. Click 'Done' when all necessary files are uploaded.
      7. Click 'Submit' to finish.

      Step 4: Check for Confirmation

      When you hit submit, an email confirmation will be sent to your FIU email. Please make sure to actively monitor your email to confirm your form was submitted.

      Certain forms can be filled out and submitted entirely in MyFIU.

      1. Log in to MyFIU.
      2. Select the 'Student Tools' tile, then 'Student E-Forms.'
      3. Then choose the form you need and follow the form instructions.
